I give, bequeath and devise (the sum of, or the following property, or percentage of my/our estate: ________________________________________ to the Heritage Christian Services Foundation, Inc., as trustee of the Heritage Christian Services, Inc., Pooled Trust to be held and administered according to the terms of such trust for the benefit of ____________________ during his/her lifetime. Upon ________________ ‘s death the remainder shall pass to the Heritage Christian Services Foundation, Inc.
